To clarify, the academic year for the Middle Level Health Provider (MLHP) means students who studied in the 4th year of B.Sc. Nursing and 2nd year of Post B.Sc. Nursing in 2019-2020 or later and passed the course.
OR,
- B.Sc. (Nursing) / Post Basic B.Sc. (Nursing) / General Nurse and Midwifery (GNM) passed candidates who have completed the Certificate Course in Community Health (CCH) through IGNOU/ other State Public Health/Medical Universities as per the curriculum of the Ministry of Health & Family Welfare, Govt. of India (specified for CHOs by the Ministry of Health & Family Welfare, GOI).
Age Limit
As of 01.04.2025, the age limit is:
- Minimum Age: 21 years
- Maximum Age:
- Unreserved/EWS (Male): 37 years
- Backward Class/Extremely Backward Class (Male & Female): 40 years
- Unreserved/EWS (Female): 40 years
- Scheduled Caste/Scheduled Tribe (Male & Female): 42 years
- Age Relaxation:
- For disabled (Divyang) candidates, the maximum age limit can be increased by 10 years as per the Bihar Government rules.
- Departmental candidates can get a maximum age relaxation of 5 years if they meet the basic requirements, as per the State Health Society, Bihar, Human Resource Rules-2021.
- According to the Bihar Government rules, candidates who were within the maximum age limit between 01.01.2022 and 01.04.2025 are also eligible, if they meet the other qualifications.
Please note that you can only get one type of age relaxation.

You can only pay the application fee online. You can pay through Internet Banking/Credit Card/Debit Card/Unified Payment Interface (UPI). The application fee is:

- A fixed monthly remuneration of Rs.32,000/-.
- Performance-linked payment of Rs.8,000/- per month, based on the calculation of various health indicators.
